Individual Therapy:  Self-pay/Private Pay rate: $130 - Sliding scale options if needed

- Independence Blue Cross

Ohio:

* I do not accept any Medcaid or Medicare plans at this time*

Insurance plans would need to be verified before your session as well as any consent forms and intake paperwork.

It is your responsibility to know your insurance plans benefits

Late cancellation/no-show fee: $60 (Insurance will not cover this)
